How to Initiate Goals of Care Discussions With Family

Image
?Initiating a goals of care discussion with family members or professionals ensures that your wishes for daily living and eventual care needs are planned and executed according to your wishes. While many adults set these conversations aside until a health diagnosis or other situation occurs, having goals of care discussions early in life allows more effective planning. Why Have Goals of Care Discussions With Family If you are an aging parent who expects your adult children to care for you when the time comes, these discussions are critical. In my meetings with aging adults, many parents say their children will care for them. However, few have asked the question. My advice is to ask early and to be practical. Sometimes, individuals and families fail to initiate conversations because they fear others will not meet their needs. Adult children invested in building a career, married, and have children may have little time to care for you. Some parents do not want to burden children b

?I?ll Take Care of It?

To succeed in today’s workplace, people need to demonstrate their value and curtail any hint of dispensability. At a minimum, they need to at least act the part. Here is a personal example to illustrate the point. I needed help to remove a piece of furniture from my apartment. I asked Joe, who works in my building, if he’d like to make a little extra money by assisting me. Sighing, Joe went into this long dreary story about how much effort this would take. After all, didn’t I know about the rules of how to dismantle the furniture for recycling, the rules about when large items went curbside for pick up, and the rules about how to store furniture in the basement without creating a fire hazard? Not only was Joe cranky (and unappreciative), but he turned everything into my problem. Needless to say, I was relieved that he turned down my request, because who wants to work with someone who has that kind of attitude. Fortunately, Adam was referred to me. When he arrived at my apartment t

For Immediate Release: Dateline: East Greenwich , RI My Wife and I are on AA 461 on the way to Las Vegas today. In first class (which is really a business class, 20 seats on the small A321, but still better that Europe’s domestic business class with tiny seats three-across) and the meals they offered were a stuffed bell pepper or a spicy chicken taco. About half the meals were picked up half-eaten or uneaten. What is the airline brass thinking? Why not something simpler for more of a general palate, like meat and fish? (I know you can order special meals, but trying putting in the time to do that before your plane departs next week. Jet Blue, of course, provides a selection of three options from five choices every time.) The plane was new, the service excellent, and I realize we’re not on board for a culinary experience. But IF the airline chooses to offer food, then why not make the choices palatable to as many people as possible? If you want vegan or orthodox or food grown from

Kids, X-Rays, CAT Scans, and Cancer, Oh, My!

Detecting and treating specific diseases may require radiation-sensitive devices, and deciding on the risk/benefit ratio can be difficult. Photo by National Cancer Institute on Unsplash The monthly medical meeting was uneventful, with the usual information being dispensed under department regulations. All the healthcare providers waited for it to be over until one pediatrician stood up to tell the group something that troubled him. " The other day, a mother and her child came into my office ," he said, " and she praised the local clinic where her child had been receiving treatment. She said they were so good and so careful that each time she brought her child in, they took an x-ray of his body. " This might have happened several times a year. The physician turned and looked at the group, waiting for some response. There was none, and it seemed perfectly fine with everyone that this child was repeatedly being exposed to needless X-rays . It was an example of

We get to the curb, check our bags, standard GREAT skycap service. It’s Saturday night, and we’re flying to Las Vegas for a weekend of fun. Go inside to the First Class/Dividend Preferred line. That’s the place US Airways has set aside for their BEST customers. There is only one agent helping the customers in First Class (several other agents are down about 50 feet in coach), and she has just finished up with someone. I’m next. I’m ready. She spends a few minutes folding documents, chatting away with the airport security officer, folding more papers… NEVER LOOKING UP. About 6 minutes have gone by and we have yet to be acknowledged. She finally begins counting money, by this time my partner and I are getting annoyed. My partner, the least volatile one on or our team, goes up to the counter and politely asks the ticket agent if she is still open. Paula D. states that she is “closed” and that someone else will be helping us and goes back to counting and folding. NOTE: We have been sta

Can a Dinner, Free Pizza, or Square Dancing Beat Ethics in Alzheimer?s Drug Trials?

Image
Alzheimer’s disease is a killer, and it tortures the patients and their families, but finding a “cure” presents questionable drug trial ethical concerns. dimaberkut @ 123RF.COM Advances in healthcare and medicine are only possible through the use of clinical trials with volunteers who agree to participate, and it is this cohort that will determine whether or not a medication is acceptable to proceed to prescription for patients. In addition to the usual volunteers who are recruited from the pool of individuals selected, some are "professional" participants who watch for clinical trials and then volunteer for them. Unfortunately, these individuals, who spend their weeks, months, or years in clinical studies, receive little attention in the professional literature. However, they are among the healthy volunteers who come forward and would not be denied entrance if they met the study's required entrance/exclusion criteria. I've known sleep studies in which indiv

America is crying out for a Jim Lehrer

Image
Throughout its history, America has faced one monumental problem after another. As such, I don’t know if there has ever existed a period in this nation that could be considered idyllic. But as a new immigrant in the 1990s and early 2000s, I found an atmosphere in this country that was as close to ideal as could possibly be, relative to the environments that I had experienced before then. I really want my early years in America back. Because of the extreme polarization of American politics and the dangerous cleavages it has created in our society, one of the people I find myself missing the most nowadays is Jim Lehrer, the longtime anchor of the PBS NewsHour. He was the embodiment of perfection in journalism. His voice was so respected by Americans that he was chosen to moderate as many as twelve presidential debates in the period between 1988 and 2012. One major driver of the divisiveness in America today is the near-total distrust of the national media. It is the nature of most pol

From Larry Hass: I am so pleased to be back performing at Washington Magic on November 8, 2024. It has been a few years since I was on the show there. Since then, I have been performing and teaching in Las Vegas with Jeff McBride. I also had a two-month, extended run of my solo show, Magical Life, in Chicago at the Rhapsody Theater. So, on November 8, I will be performing new routines I developed for the Chicago Show. I hope the Washington Magic audience will find them artistic and astonishing! One thing I love about Washington Magic is the wonderful people who come out to the shows. I look forward to seeing old friends in the audience and new ones, as well! ?LETHAL DOSES?Wins First Place in the FIREBIRD Book Awards? True Crime Genre

Image
"Riveting, brilliant, unputdownable. It's one of the best books I've read this year." —Jan Brogan , author of The Combat Zone, Final Copy Denver, CO — LETHAL DOSES by John Madinger has won 'First Place' in the FIREBIRD BOOK AWARDS' True Crime genre .  Authors and publishers from around the world submitted their work to the International Firebird Book Awards for consideration. A select panel of 25 judges independently scored and evaluated the quality of the writing and production. Only entries with the highest scores were awarded the coveted Firebird . In 2024, fentanyl kills more than 200 Americans every day, and it gives way to a complex history that leads back to one man: George Erik Marquardt. Following a tragic weekend of fentanyl-related deaths in 1991, Marquardt was interviewed for hundreds of hours after a three-year chase by the DEA. Award-winning author and former undercover agent for the Oklahoma Bureau of Narcotics, John Madinger, reveal

You Can Fool All of the People All of the Time, Sometimes

Image
For Immediate Release: Dateline: East Greenwich , RI I’m guessing that about 90% of the statistics thrown around on social media are simply made up: “Over 90% of workers in large businesses are alienated and disengaged.” “Most people leave companies because of toxic environments.” And on and on it goes. These are picked up by others and exaggerated further. Someone said this morning that Trump appearing at a MacDonald’s “angered most of the rest of the world.” How on earth would he know that? (Note that I admit I’m “guessing” about the percentages.) The problem isn’t just the totally phony statistics, it’s that, as it is with many lies, the perpetrators tell them so often that they believe in them. And since they wrap themselves up in the confirmation bias of social media and interact solely with people who share their views and biases, they believe they’re reporting the truth. Even scientific investigations and disciplined surveys are often wrong. The famed “Hawthorn Effect,” se

Alan Weiss?s Monday Morning Memo? ? 10/21/2024

For Immediate Release: Dateline: East Greenwich , RI Monday, October 21, 2024   This past Thursday and Friday Maria and I drove four hours up to Vermont and then back again to experience falconry, a gift from my son for my last birthday. (I’m told I’m hard to shop for. My wife once arranged for me to operate an excavator in Vegas and my daughter gets us show tickets.) Niko, the falcon, was fascinating. (If you have access to Facebook, go here— https://www.facebook.com/ RockStarOfConsulting —and scroll down a short bit to the photos and movie. You’ll also see Avi the Owl.) The bird would fly to one of the wooden platforms, wait for a cue, and then fly to my wrist, snag a treat, and wait for instructions. However,  Niko also “freelanced” and would occasionally take off on his own and land in the field to snag a cricket or some other morsel he saw from 40 yards away. He will also land only on a leather glove, never on a hand, shoulder, or head. Birds of prey can lift half thei

Dashboards, Portals and Data-driven Decision Making

Image
For Immediate Release: Dateline: Toronto , Other Sunday, October 20, 2024   One of the holy grails of big data is “the portal” (also known as “the dashboard”): there is transparency, timeliness, and nifty graphics that expose never-before-seen insights. For those with the swankiest implementations, data is even personalized: depending on your level and your functional responsibilities: what you see is precisely what you need. (We’re assuming that most organizations have at least something online that tracks OKRs/KPIs/other data/etc.) So two tough questions for those who think that the job is done once it is set up: Is the implementation of a dashboard also the end of thoughtful analytics? Is it the beginning of the worst kind of management complacency? Consider the arguments: When the dashboard is developed, decisions are made about what is tracked, as well as the priority of data placement. But as time goes on, are these still the precise markers that should be tracked?
